How It Works
We will be accepting online applications through Google Forms as of September 15th, 2021. The application submission process should take no longer than one hour to complete. Please do not exit the application until you have finished and selected "submit". It will not save.
The file name should include the name of your organization and the year. (Example: BeaconHillCivicAssociation_2021)
As the system requires, you will be prompted to sign-in into your Gmail account in order to upload any materials to the application.
